How to Apply:

To apply for our behavioral health services, please follow these steps:

1. Email ashleybchildhoodadvocate@gmail.com 
2. In subject type "BEHAVORIAL SERVICES"
3. In message, please leave your name and best contact numbers.
4. Ashley B. will contact you within 48 hours from email sent. We will schedule best time to meet to complete application. Application must be sign by parent or guardian of client under 18 years of age. You will need applicants' basic information including active Medicaid card info and social security number. After application is processed, Parent/Guardian will be contacted for an initial assessment for applicant. Therapy sessions are available.
